As a permanent member of the Social Support program you will assist older people to stay at home for longer by providing the support they need. 

Reporting to the Manager Commonwealth Home Support and Carer Programs the Social Support Leader shall work in collaboration with other members of the Social Support team to provide a high quality community and centre based program of activities maximising individual choice and aspirations.

 Social Support aligns with the Wellness and Reablement model by providing education and activities which promote independence, social connection, health and reablement.

To be successful, you will require;

  • Certificate IV or Diploma level qualification in a relevant health field is desired
  • A current Working with Children Check
  • Current First Aid Certificate Level 2
  • ability to drive the Social Support bus
