Seniors Lifestyle Victoria Coordinator

As the Seniors Lifestyle Victoria (SLV) Coordinator your role with involve delivering care coordination assessments, education and support to clients over the age of 65 who are experiencing mild to moderate frailty. The SLV program aims to reduce avoidable emergency department presentations and avoid hospitalisation through targeted goal setting with participants, developing strategies to empower, build confidence and ability to navigate the service system.

Your work will also strengthen integration and collaboration between primary health, acute care services and community care services.

Who we are looking for;

  • Someone with high level of computer literacy and proficiency in use of Microsoft programs and the internet.
  • Someone who has high-level problem-solving skills and the ability to demonstrate a high level of initiative in identifying a range of alternative solutions to complex situations and tasks.
  • Someone with demonstrated skills in assessment, intervention, care coordination and discharge management enhancing participant self-management.
  • Someone with excellent interpersonal skills and the ability to communicate effectively.
  • Someone ideally with an Allied Health qualification.
  • You will be adaptable, empathetic, resourceful and be able to uphold the LCHS values - Providing Excellent Customer Service, Creating a Successful Environment, Providing Personal Best and Acting with Utmost Integrity.

To be successful, you will require;

  • Victorian Driver's Licence
  • A current Working with Children Check
