What you’ll be doing in this role?

The Healthy Mother’s Healthy Babie’s Outreach Practitioner supports vulnerable parents during pregnancy to provide them with the greatest opportunity to succeed at parenting their new baby.  The role varies with each family you work with however it can include linking families into support services that can set them up for success e.g. housing services, AOD services and material aid services, ensuring the women are linked into receive antenatal care and provision of transport to these appointments as required, provision of health coaching and education to prepare the parents for the birth of their baby.

Your work will include:

  • Identifying the needs of vulnerable pregnant individuals
  • Tailor support services to meet their individual needs
  • Liaising with external agencies to ensure the pregnant individual is prepared for the birth of their child and parenting
  • Providing health coaching to pregnant individuals around healthy lifestyle choices eg diet, smoking, alcohol and drug consumption
  • Supporting clients to attend relevant medical and support service appointments

What you'll need: 

  • Bachelor or Diploma of Nursing, Bachelor of Social Work
  • AHPRA registration or equivalent professional membership/registration
  • A current Victorian Drivers Licence
  • A current Working with Children Check (Employee) or VIT Registration
