What you’ll be doing in this role?

The Care Finders program will provide specialist and intensive assistance to help people in the care finder target population to understand and access aged care and connect with other relevant supports in the community.  The program will drive assertive outreach activities to all clients across the wider Gippsland area, including rural and remote communities.

 

Your work will include:

  • Support senior community members, within the target group to understand and access aged care and connect with other relevant supports in the community.
  • Undertake engagement and network building with the local community, care services and community providers to support both identification of and engagement with potential clients and available services.
  • Facilitate client linkages and referrals as appropriate to aged and community services.

 

What we’re looking for:

  • Tertiary qualification in Social Work, Aged Care or Community Services (desirable)
  • A current Victorian Drivers Licence
  • A current Working with Children Check (Employee)
