Are you ready for a new professional challenge? This exciting opportunity calls for a self-motivated and passionate Pharmacist to develop this new position within the Chronic Disease Management (CDM) team.
Our integrated service model will continually expose you to other health professionals. It means you’ll provide your specialist expertise as part of a holistic solution for patients. You’ll be exposed to more learning opportunities to enhance your career, while gaining deep satisfaction from witnessing the results of your talents and effort in your patients.
Your role will include developing the most appropriate service model and defining the role of Pharmacist at LCHS utilising best practice evidence and internal stakeholder engagement. You will provide a high level of clinical care, counselling patients on the use of their medications as necessary. You’ll enjoy an environment where new ideas and initiatives are welcomed and supported.
This role would suit a clinical or hospital pharmacist interested in making a difference in a community care setting and working in a multidisciplinary team.
To be successful, you will require;
- Bachelor Degree of Pharmacy
- Current AHPRA Registration
- Current Victorian Driver Licence
- A current Employee Working with Children Check
