As the Seniors Lifestyle Victoria Coordinator, you will be using your individual expertise, people skills and ability to multitask and move with change to assist clients referred to the program. You will provide guidance and support to link clients into supports and or services that best meets their need and builds their engagement in their own wellbeing. You will help build their individual capability and capacity whist connecting them with community and mainstream services appropriate to their need and situation.

Who we are looking for;

• Someone with great people skills, excellent written communication and time management skills
• Someone with demonstrated experience & expertise who can engage with seniors from various backgrounds. Experience working with people who are experiencing mild to moderate frailty who may be at risk of admission into tertiary care facilities, including hospitals
• Experience with My Aged Care Portal and processes
• You’ll be adaptable, empathetic, and resourceful and be able to uphold LCHS values – Providing Excellent Customer Service, Creating a Successful Environment, Providing a Personal Best & Acting with Utmost Integrity

To be successful, you will require;

• A current Working with Children Check
