Join our not-for-profit team as a Senior Care Partner supporting older people to live safely, independently, and confidently in their own homes.


Based in Wonthaggi, this full-time role combines office-based work, home visits across the community, and supporting the Care Partners in the team. 

You’ll manage a caseload of participants receiving Support at Home services, completing assessments, care planning, reviews, and coordinating tailored services that put participant choice first.

To be successful, you will require;

  • Appropriate Qualification compliant with Australian Qualifications Framework (e.g. Diploma of Case Management and/or Community Services /Nursing/ or 
    related field, or 3 or 4 year undergraduate degree in a health related field) 
  • Experience in Support at Home, case management, aged care, community services, nursing, or a related field
  • Strong relationship-building and problem-solving skills
  • A passion for client-centred care and helping people achieve their goals
  • A current Victorian Driver’s Licence
