We are looking for experienced and enthusiastic team players to be a part of the new Care Finders program. We are looking for professional individuals with a passion towards supporting our communities, providing linkage services and check ins for those identified as in need.
The Care Finders program will provide specialist and intensive assistance to help people in the care finder target population to understand and access aged care and connect with other relevant supports in the community. The program will drive assertive outreach activities to all clients across the wider Gippsland area, including rural and remote communities.
To be successful, you will require;
- Bachelor degree (social work, human services, aged care, community services or health) and/or relevant experience
- Discipline specific registration where required.
- Victorian driver’s licence
- Post graduate qualification in health, aged care and/or related field (Desirable).
