Care Partner Team Leader

We are currently seeking a Care Partner Team Leader.
As a Care Partner Team Leader, you will play a pivotal role in overseeing a team of dedicated Care Partners, ensuring the provision of excellent care services to our consumers. You will be responsible for managing and coordinating the day to day operations of the Care Partner team, providing guidance, support, and training to maximise their performance and ensure exceptional care delivery.
Your work will include:
- Lead, supervise and motivate a team of Care Partners, providing guidance and support to ensure the delivery of high-quality care services.
- Coordinate and allocate workload effectively, ensuring efficient use of resources.
- Conduct regular supervisions and provide constructive feedback, mentoring, and training to enhance individual and team performance.
- Maintain open communication channels with team members, fostering a positive and collaborative work environment
- Oversee the recruitment and on boarding process for new Care Partners.
- Address and resolve client concerns and complaints promptly and effectively, demonstrating excellent customer service skills.
- Collaborate with other departments and professionals to facilitate a multidisciplinary approach to client care.
- Stay updated with industry trends, regulations and best practices in care management and ensure team compliance with relevant standards.
- Support the development and implementation of policies, procedures, and quality improvement initiatives.
To be successful, you will require;
- Previous experience in a supervisory or leadership role within a care setting is desirable.
- Strong knowledge of care management principles, client assessment, and care planning.
- Excellent organisational and time management skills with the ability to prioritise and meet deadlines.
- Exceptional interpersonal and communication skills to interact effectively with consumers, team members, and stakeholders.
- Proven ability to motivate and lead a team, fostering a positive work culture.
- Proficiency in using care management software and computer applications.
- Knowledge of relevant regulations and standards in the healthcare industry.
- Compassionate, empathetic, and committed to delivering person-centered care
